QUESTIONS FOR DEBTOR(S) AT 341 MEETINGS OF CREDITORS BY DEBTOR’S COUNSEL

1. Please place your appearance on the record by stating your name and the law firm with which you are a member. If you are not a member of the Firm that filed the case, please state:
   1. The firm you work for, and
   2. Whether you have filed a Notice of Special Appearance and Statement of Attorney for debtor(s)
2. Indicate on the record that you have reviewed debtor’s government-issued photographic identification and have verified debtor’s identity AND
   1. that you have provided the Trustee’s office with the Declaration of Identity Form prior to Today’s hearing, or
